Here, you can submit a form telling us about your complaint against a health care practitioner.

The Division of Medical Quality Assurance (MQA) investigates complaints that are submitted to the Florida Department of Health through this portal by law when they are found to be legally sufficient. In other words, if the allegations made in the written and signed complaint are found to be true and in violation of recognized standards of practice, the complaint will be deemed legally sufficient, and the subject of the complaint (or their attorney) will be provided with a copy of the complaint.

To find answers before you start, visit the Bureau of Enforcement FAQs and see the complaint process. Remember, the Department does not investigate grievances about procedure or appointment costs, bedside manner, professionalism, customer services, or personality conflicts.

Submitting a complaint is the first step in the complaint process. Make sure you have all the information ready first, including the subject of the complaint, a description of the possible violation, and a signature, because the session will expire after 30 minutes. You can submit a complaint anonymously but contact information must be provided that can be used to complete the investigation as incomplete forms cannot be processed.

Note: The session will expire after 30 minutes if no activity. The unsubmitted data will be removed from the system when the session expires. Please make sure you have all of the information ready before proceeding.
